新诊断2型糖尿病患者血清前蛋白转化酶枯草溶菌素9水平的变化及其临床意义

摘要  目的   探讨前蛋白转化酶枯草溶菌素9(PCSK9)在新诊断T2DM及NGT人群中的差异及其与血糖、胰岛素及其他代谢指标的关系。  方法 选取新诊断T2DM患者200例和NGT者244名,EILSA检测血清PCSK9水平。 结果 T2DM组血清PCSK9水平高于NGT组 [(90.58±40.48) vs (63.09±33.83)ng/ml](P<0.01)。总人群血清PCSK9 水平与年龄、SBP、BMI、WC、TC、TG、LDL-C、FPG、FIns、胰岛素抵抗指数(HOMA-IR)呈正相关,与HDL-C呈负相关(P<0.05);NGT组血清PCSK9 水平与年龄、TC、LDL-C、FIns、HOMA-IR呈正相关(P<0.05);T2DM组血清PCSK9 水平与TC、TG、LDL-C、FPG、FIns、HOMA-IR呈正相关(P<0.05)。多元逐步回归分析显示,LDL-C是PCSK9的影响因素,总人群及T2DM患者中,FPG是PCSK9的影响因素。  结论  T2DM患者血清PCSK9水平高于NGT人群, PCSK9可能参与了糖尿病的发生发展。

Abstract  Objective To investigatethe difference of proprotein convertase subtilisin/ kexin type9 (PCSK9) level between patients with newly diagnosed type 2 diabetes mellitus (T2DM) and people with normal glucose tolerance (NGT), and evaluate the correlation between serum PCSK9 and glucose, insulin and other metabolic factors.  Methods A total of 200 T2DM patients and 244 NGT subjects were randomly enrolled in this study. Serum PCSK9 level was measured by ELISA.   Results  Serum PCSK9 level was higher in T2DM group than in NGT group [(90.58±40.48)vs(63.09±33.83)ng/ml](P<0.01).Serum PCSK9 level was positively correlated with age, SBP,BMI,WC,TC,TG,LDL-C,FPG,FIns and HOMA-IR and negatively correlated with HDL-C(P<0.05). Serum PCSK9 level was positively correlated with age, TC, LDL-C, FIns and HOMA-IR in NGT group(P<0.05); and positively correlated with TC, TG, LDL-C, FPG, FIns and HOMA-IR in T2DM group(P<0.05).Multiple stepwise regression analysis showed that LDL-C was an influencing factor for PCSK9 and FPG was an influencing factor for PCSK9 in the whole study population and T2DM group.  Conclusion  Serum PCSK9 level is higher in T2DM patients than in NGT subjects.PCSK9 may play a role in the development of T2DM.
